Incomplete Grade Contract

Students who experience extenuating circumstances that prevent them from completing coursework within a given term may request an Incomplete (I) grade from their instructor using SIS Self-Service. An approval of an incomplete grade request is neither automatic nor guaranteed.

Incomplete Grade Contract Form

Please review your school's policies before requesting an Incomplete Grade.
Follow these steps to
submit your request:

Step 1:
Communicate with your Professor (and Advising and/or Program Director when appropriate) to determine if an
Incomplete Grade Contract is appropriate for your circumstance.
Step 2:
Submit an Incomplete Grade Contract by logging into Self-Service
Step 3:
Click on the Incomplete Grade Contract form under Registration>Online Forms>Incomplete Grade Contract.
Step 4:
Select from list of Eligible Courses, Acknowledge you have communicated with your Professor, and click Continue.
Step 5:
Provide requested information and click Submit.
Step 6:
Await a decision from your Professor of Approved, Approved with Modifications, or Denied.
If Approved:
You will receive an email confirmation- No action required.
If Approved with Modifications:
You will receive an email confirmation- Click on the link provided to accept or deny terms.
If Denied:
You will receive an email confirmation- No action required.
