Johns Hopkins University requires submission of the following forms for each request permitted under the Family Educational Rights and Privacy Act (FERPA). Additional forms will be added to this list as they become available.
Please note: Students who do not authenticate using an active JHED ID when submitting FERPA request forms will be contacted to schedule a virtual appointment during which they will be asked to provide a valid photo ID as proof of identity.
